More than a place to stay — a place to be cared for
Skilled nursing is for residents who need ongoing medical care that can't safely be managed at home. At Silver Oak, that means registered nurses and certified nursing assistants on-site twenty-four hours a day, working from a care plan built around your loved one's specific needs and preferences.
What's included
- 24/7 on-site registered and licensed nurses
- Physician-directed care plans, reviewed regularly
- Medication management and IV therapy
- Wound care, diabetes, and chronic-condition management
- Help with bathing, dressing, mobility, and daily living
- Three home-style meals a day, plus snacks and dietary accommodations
Who it's for
Skilled nursing suits older adults living with complex or chronic medical needs, those recovering from a serious illness, and families who can no longer safely provide the level of care required at home. Many of our long-term residents first came to us for short-term rehabilitation and chose to stay.
Why families trust us
Our nurse-to-resident ratios, low staff turnover, and strong health-inspection history mean your loved one sees the same trusted caregivers day after day. We treat skilled nursing as a relationship, not a transaction.
The details families notice
- Consistent caregiver assignments so residents see familiar faces
- Care conferences that keep your family in the loop
- On-site therapy if recovery goals change
- Engaging daily activities, chapel services, and outings
Questions families ask
Yes. We are a Medicare-certified skilled nursing facility and also accept Medicaid, most private insurance, and private pay. Our admissions team will help verify coverage before any commitment.
In many cases we can coordinate an admission within 24–48 hours, often directly from the hospital. Call our helpline and our team will move quickly to assess needs and availability.
Other ways we support your family
What to expect during your skilled nursing stay at Silver Oak
Choosing skilled nursing is a big decision, and knowing what daily life looks like can make it easier. When your loved one arrives at Silver Oak, our nursing and care team completes a thorough intake assessment — reviewing medical history, current medications, physician orders, and personal preferences. From that first conversation, we build an individualized care plan and assign a primary nurse who becomes the main point of contact for your family.
Each day includes structured meals, social activities, medication administration, and regular clinical check-ins. Physician visits are coordinated on-site, and our team communicates proactively so families never feel out of the loop. If goals change — whether a resident needs short-term rehabilitation or additional support — we adjust the plan without disruption. Medicare, Medicaid, and most major insurance plans are accepted; our admissions team verifies coverage before day one so there are no billing surprises.
We serve families across Marion, Iowa and the surrounding communities. Call (319) 377-7363 to speak with our admissions team any day of the week.
